On behalf of myself, and my child or ward, ("Applicant") and on behalf of our representative heirs, and personal representatives, I hereby release, waive and forever discharge Coerver Coaching Camps, Affiliates, and all employees, Officers, Directors, Agents, and volunteers, (The Releasees") from and against any and all liabilities, claims, demands, actions, causes of action, damage costs or expenses for personal injury, (including without limitatin) bodily injury and/or death, and/or property damage which I or the Applicant may sustain and which are or may be caused by the act or omission of the Applicant, his/her fellow players, or any of the Releasees except where the Releasee's act or omission constitutes gross negligence or willful misconduct. I agree to indemnify the Releasees from any person injury or property damange caused by the gross negligence or intentional act of Applicant with participating in the Coerver Coaching Camp. |
